See How Learners Reason, Not Just What They Conclude
2-Sigma Reasoning is an AI virtual patient platform that we developed at the University of Cincinnati College of Medicine over four years, in which a participant interviews, examines, works up, and manages a simulated patient who responds to each decision and then receives written feedback from an AI supervisor on how they reasoned.
Because every encounter produces an action-level transcript, the platform records the sequence of decisions that led to a diagnosis, which current assessment captures only under direct observation.
When we analyzed second-year medical student encounters with natural language processing and machine learning, we found that specific actions discriminated correct from incorrect diagnoses and that students who reached the same correct diagnosis followed recognizably different routes, an initial signal of stable reasoning patterns, or phenotypes, that a single-institution student sample cannot confirm.
